Any player participating in try outs for a travel team MUST complete a waiver and hand to a Plymouth Soccer Club representative at the registration tent.  If you do not attend the first tryout please give waiver to the coach of the age group.  The waiver which is in Microsoft Word format has input fields for easy completion.

MEDICAL RELEASE FORMS

Any player who is participating whether it is at the travel or intramural level must complete a Medical Release forms for each season.  The forms are in either a Microsoft Word Format (.doc) and Adobe Acrobat Reader Format (.pdf).
